The Florida Department of Health in St. Lucie County offers the CDC‑recognized National Diabetes Prevention Program.

This year-long lifestyle change program supports individuals at risk for type 2 diabetes through structured coaching, group support, and skill-building sessions focused on nutrition, physical activity, and sustainable behavior change. Participants meet regularly with trained lifestyle coaches who help guide goal setting, track progress, and build momentum toward healthier habits.

Sessions are conducted in a hybrid format, meaning that participants can join sessions either in person or virtually.

Glucometer Distribution Initiative

We provide free glucometers to eligible community members to support early detection and daily diabetes self‑management. Participants receive instructions on using the device and understanding results to help improve early detection and daily diabetes self-management.

Our trained health educator offers the following presentations for schools, workplaces, and community groups:

This fun, interactive activity promotes simple preventive habits like drinking water, adding steps, reducing sweets, and practicing stress relief. Participants can complete it at their own pace and celebrate progress as they go.
