Florida CHARTS

Florida’s Community Health Assessment Resource Tool (CHARTS) is a one-stop website for public health statistics and community health data in Florida. It provides access to a wide range of information, including health statistics like births, deaths, disease rates, population data, and behavioral risk factors


Healthy St. Lucie 

Healthy St. Lucie is a community‑led coalition of 36 partner organizations working together to improve health and well‑being across St. Lucie County. The coalition leads the full Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) process, ensuring that local voices guide every step.

Our work focuses on three shared priority areas: Chronic Diseases and Conditions, Mental and Behavioral Health, and Health Literacy. These priorities were identified through extensive community input and data review.

Although the coalition is now community‑driven, the Florida Department of Health in St. Lucie County remains an active and engaged partner.

Together, we work to ensure that every resident has the opportunity to live a healthy, connected, and thriving life.
